Memorandum of Agreement
Norwalk Economic Opportunity Now, Inc
and
George Washington Carver Community Center

Services: Support The Carver Youth Development Program and connect Carver’s low-income families to NEON’
s Human Services Infrastructure information and referral (I&R) system.
Objectives: 1) To enroll at least 80 families whose children participate in the Carver Summer Camp
Program into NEON’s HSI system.
2) To enroll at least 20 families whose children participate in the Carver Youth Development Program into
NEON’s HSI system.
3) To refer at least 100 additional Carver clients (individuals or families) to NEON for I&R, DSS pre-application
assistance, case management services or other needed services.
4) To have a minimum of four (4) events where NEON HSI staff can present HSI services to Carver families
and/or individuals.
5) To refer any Carver clients or participant’s parents who can benefit from Connecticut Department of Social
Services programs to NEON HSI staff for DSS pre-application services offered by NEON staff.
6) Support children enrolled in The Carver Youth Development Program who are from families who ar at or
below 150% of the federal poverty level.
Roles and Responsibilities
NEON, Inc.
• The NEON Executive Director agrees to purchase services from the Carver Community Center to support the
Human Services Infrastructure system to be connected to Carver clients and families.
• NEON will render $45,007 in payment for facilitating and accomplishing the above objectives (1 through 5).
Four equal quarterly payments will be released to the Carver (May 15, 2008; August 15, 2008; November 15, 2008;
February 15, 2009) after the NEON Adult Services Planner receives a completed programmatic report indicating the
progress and outcomes associated with the above objectives. The quarterly reports are due on the 30th of the
following month after end of each quarter (April, July, October, and January).
• NEON will monitor this HSI agreement twice a year. The Adult Services Planner will give a fifteen-day notice to
the Director of the Carver of a pending monitoring visit.
• NEON will give to the Director of the Carver the contact information for all the services NEON provides in order
for Carver staff to make referrals.
• NEON will provide staff to at least four (4) Carver events to present NEON services and activities that are
available to area residents and families.
Carver Community Center
• The Carver will provide the means to fulfill the above objectives (1-5).
• Carver staff will facilitate HSI intakes with those clients represented in objectives 1 and 2 above. These
intakes will be kept in the client’s file and a copy will be sent to the NEON Adult Services Planner.
• Provide to NEON, Inc. Universal Intakes on each qualifying youth enrolled in the Youth Development Program.
Each quarter, The Carver will present to NEON’s Adult Services Planner, a YDP report containing the student
names, their GPA, weekly attendance, and income eligibility (to satisfy CSBG requirements for 150% of poverty or
below), and the student’s date of entry into the program and the student’s to-date status. This is a cumulative
report, indicating each quarter any new students. The Universal Intakes for newly enrolled YDP students will
accompany this quarterly report.
• The Carver will be responsible to submit quarterly HSI reports that summarize the accomplishments and
fulfillment of the above objectives. These reports are due the 30th of the first month following the end of each
quarter (April, July, October, January). Submission of the quarterly report will be considered the quarterly invoice for
funds that support this agreement.
The execution and fulfillment of this agreement is contingent upon NEON, Inc. receiving the necessary funds from
the agency’s Community Service Block Grant and Human Service Infrastructure contracts with Connecticut’s
Department of Social Services.
